Scope of Service: The Nurse supervisor job description clarifies the roles in Continuum of Care Veterans’ programs. ROLE DEFINITION

Responsibilities of the Nurse Supervisor

  • Coordinates daily assigned administrative tasks with schedule, liaise with administrators and staff, and communicate any change of protocol to supervisor or nurse manager.
  • Coordinates and liaise with pharmacy, APRN’s, and case managers as needed.
  • Engages in patient admissions and transfers as well as discharges.
  • Assist residents providing compassionate, respectful, and ethical care.
  • Ensures medication refill drop off and pick-ups are completed on a day-by-day basis.
  • Reports any medication errors, mishaps, or shortages to nurse manager or DON.
  • Supervises the nursing staff including, completion of performance supervisions.
  • Reviewing and provides consultation to policies relative to nursing responsibilities.
  • Communicates to nurse manager or DON any problems that arise from medication pick up, incomplete medication orders or absence of medication orders. (Nurse manager or DON will report to VA teams).
  • Takes on-call responsibilities.
  • Observes client’s ability to self-administer medications while educating resident on procedures.
  • Undertakes training, support and mentoring of new staff and gives guidance.
  • Collaborates with interdisciplinary team to maintain positive patient progression.
  • Collects and document baseline data of each resident upon admission.
  • Completes rounding and quick assessments of each resident and periodically during shift.
  • Observes with each resident interaction to assess physical and mental status and condition.
  • Dispenses and/or administers medications as needed and ordered.
  • Reports to Nurse Mgr. anything that does not “seem right” with any resident.
  • Responds to emergencies calmly and efficiently by identifying them and acting accordingly.
  • Assists w/changing monthly client MAR/MOR.
  • Maintains and updates (if needed) all CoC documentation forms after approval from QI-VP.
  • Provides crisis intervention and support as clinically indicated.
  • Understands and abides by VA/CoC - SOP protocols for after hours.
  • Motivates team members to accomplish team goals.
  • Ensures that all medical documentation, medical history, and files on each client abides by HIPAA compliance rules and regulations.
  • Follows and abides by National League of nursing (NLN) and Nursing Practice Acts (NPA) guidelines, laws, and restrictions.
  • Assumes responsibility for patient care that is not outside the scope of practice.
  • Maintain compliance with all safety and health regulations.
  • Manage stress effectively and utilize self-care.

  • Candidate should have a flexible schedule and be able to work mornings, evenings, weekends, holidays, and on call schedules.

Environmental Responsibilities:

  • Assists in maintaining a safe and secure environment for staff and clients.
  • Maintains a clean, well-organized work environment.
  • Notifies supervisor of maintenance and/or safety issues and generates work orders as needed.
  • Participates in fire and emergency and all health and safety drills.
  • Completes other designated environmental tasks as assigned.
  • Quality Assurance:
  • Adheres to policies and practices as outlined in staff handbook and program operations manual.
  • Consistently works in a respectful, constructive, and cooperative manner.
  • Upholds all Health Insurance Portability and Accountability Act (HIPAA) regulations and secures all necessary releases of information for outside communication.
  • Models culturally competent behavior interacting with clients, providers, and fellow staff.
  • Adheres to the standards of the Council on the Accreditation of Rehabilitation Facilities.
  • Participates in agency committee work when available or as permitted.
  • Completes all trainings as required per program contract and agency policy. Provides training as necessary team and agency members.
  • Attends agency sponsored in-services, outside trainings, conferences and meetings.QUALIFICATIONS
  • Bachelors of Nursing degree with three to five years clinical nursing experience: or Associates of Nursing, with 5 to 10 years clinical nursing experience.
  • 1-3 years of managerial experience in nursing.
  • Has current and ongoing knowledge of psychotropic and medical medications and effects on persons served.
  • Prior experience (preferred) working with people with mental illness and substance use disorders.
  • Valid Connecticut Driver’s license is required. Reliable transportation and willingness to transport clients in personal vehicle that hold valid liability insurance.
  • First Aid/CPR/BLS certification or willingness to obtain certification within 90 days of hire. Employee is responsible to maintain ongoing certification.
  • Annual physical, PPD, and other health requirements may be required by Continuum and the Department of Public Health and/or contracted agent.
  • Proficient computer skills required, including Microsoft Office and electronic record keeping.

Job Type: Full-time

Pay: $96,600.00 per year
