The Opportunity: We are seeking to hire a Community Liaison to join our CCBHC Team. This individual will assist with developing and maintaining collaborative relationships with community partners, including, but not limited to Oriana House and Summit County DD Board. They will provide time limited care coordination and case management for clients engaged with Oriana House and Summit County DD Board.

What You’ll Do:

  • Develop and maintain collaborations with community entities, including Oriana House and Summit County DD Board.
  • Serve as a liaison with clients who are also receiving services from Oriana House programs.  For clients who are working with established case management teams, the Community Liaison serves as a point person for Oriana House to support coordination between community corrections and the outpatient treatment providers.  For clients who are not working with established case management teams, the Community Liaison assists with time limited case management and care coordination activities.
  • Maintain weekly contact with Oriana House programs to review upcoming scheduled psychiatric, primary care, and other appointments.
  • Serve as a liaison with the Summit County Developmental Disabilities Board (DD Board), including being aware of resources available from the DD Board and knowledgeable regarding referral processes and care coordination.
  • For clients who are not working with established case management, provide time limited case management.  Link clients in need of additional services with other organization programs and Summit DD resources.
  • Participate in the monthly MI/DD meeting to support the shared individuals served by Summit DD and the organization.
  • Participate in staffing meetings with clients, family members, guardians, and Summit DD Board as requested.
  • Provide time-limited skill building and resource acquisition support to clients with assessed need for such interventions.
  • Conduct outreach efforts, as applicable, with clients/patients who do not follow-up with scheduled appointments.
  • Connect family members to the Family Coordinator and Outreach Specialist, as applicable.
  • Identify potential barriers to services (i.e., transportation, technology) and implement a plan to address identified barriers.
  • Available to the individual to provide, coordinate and/or assist in crisis intervention and stabilization services during regularly scheduled working hours.
  • Maintain relevant documentation and provide statistical reports in accordance with agency documentation standards.
  • This position is part-time (20 hours a week) working normal business hours.

What We’re Looking For:

  • High School diploma required. Bachelor’s Degree in Psychology, Social Work, Sociology, or a related field and prior experience preferred.
  • In lieu of a Bachelor’s Degree, at least 3 years of relevant full-time employment experience working directly with individuals with severe and persistent mental illness required.
  • A car and a valid Ohio driver’s license and insurability through the Agency’s insurance carrier required.
  • A deeply held belief in CSS’s recovery-centered mission.
  • Ability to develop positive relationships with individuals with mental illness, developmental disabilities, and substance abuse challenges.
  • Skill in assessing individuals for strengths, needs, abilities, and preferences.
  • Excellent organizational, attention to detail and communication skills.
  • Ability to work in a team environment.
  • Proficiency in Microsoft Outlook.
  • Skill in managing time and meeting deadlines.
  • Ability to negotiate assertively from an advocacy perspective.
