Care Coordinator II

Pay: $20 per hour
POSITION SUMMARY
This position reports to the Specialty Care Coordination Manager and assists patients to access needed specialty medical services by providing a link between patients, their primary care providers and participating specialty providers and other tertiary services.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides program support by entering accurate and complete data into the database.
- Provides monthly review of new patient files as an internal check to assure data integrity.
- Maintain strict confidentiality of patient information.
- Ensure individual providers are not overburdened serving Project Access Northwest patients.
- Maintains the patient’s application and referral files.
- Assist with assuring sufficient patient and office materials are available as needed.
- Communicates regularly and effectively with referring clinic staff and Project Access Northwest team members.
- Points out system or process problems when they become apparent and engages in team problem solving.
- Is available to help others as work levels and priorities change, at supervisor’s discretion.
- Develops good working relationships with internal and external customers.
- Continually stresses customer service and satisfaction in all activities.
- Demonstrates a commitment to the mission, core values and goals of Project Access Northwest and its values of health care access for un- and underinsured.
- Perform other duties as assigned by Manager.
SECONDARY DUTIES AND RESPONSIBILITIES
Attend staff meetings, in-service meetings as required.
SUPERVISORY RESPONSBILITIES
This position has no supervisory responsibilities.
QUALIFICATIONS AND REQUIREMENTS

BS preferred, or up to 2 years related experience: or equivalent combination of education and related experience. Has previous case manager, referral coordinator, or care coordinator experience and good attention to detail. Understanding of commercial and Medicaid Insurance helpful.
Communication Skills
Ability to read and comprehend instructions and write basic correspondence such as memos. Data entry skills with a high degree of accuracy. Bi-lingual Spanish preferred.
Reasoning and/or Critical Thinking Ability
Work situations require consideration and interpretation of circumstances or information to choose the most effective response. Solutions may be technical, yet relatively straightforward and well-defined once problems are understood.
Computer Skills
Must be able to use and understand Microsoft Office programs, Word, Excel and Outlook. Experience with EMRs or EHRs helpful.
Licenses or Certificates
Medical Terminology preferred
Physical Requirements and Working Environment
This position requires the manual dexterity sufficient to operate phones, computers, and other office equipment. This person must speak clearly using the English language to accurately convey information and be able to hear at normal speaking levels both in person and over the telephone. Vision correctable to 20/40 is required to review written materials. The working environment is a typical office environment.
